ENROLLMENT CRITERIA

  • I take responsibility for any vandalism or damage my teen(s) may cause and that I will pay for any damage if needed.
  • I take responsibility for any inappropriate behavior, as determined by the Temple Beth El team. I understand that I may have to pick up my teen(s) immediately from wherever the program is located.
  • I will keep the Temple informed of changes to my contact information.
  • If I cannot be contacted in the event of an emergency, I hereby grant permission to
    authorize any emergency action necessary to ensure the safety of my teen(s).
  • I agree to indemnify and hold harmless Temple Beth El and all of its employees and Trustees, if any injuries occur to my teen(s) during any event at the Temple, while in transit to/from any Temple program, or while at any Temple-related off-site event, unless such injuries are as a result of the gross negligence or willful misconduct of the Temple or any of its employees.
I understand that my teen(s) picture (without names) may be included in temple publications.
